About The Position

The Licensed Practical Nurse (LPN) will administer medications safely and accurately, assist the RN with patient assessments, and implement appropriate rehabilitation nursing interventions. This role involves performing Quality Improvement assessments, delivering care based on patient response, managing pain, and developing individualized teaching plans. The LPN will contribute to a safe, patient-centered care environment within an interdisciplinary team.

Requirements

  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
  • Current state licensure as a Licensed Practical/Vocational Nurse.
  • CPR/BLS certification required.
  • Minimum one year of Medical/Surgical experience in an acute care setting preferred.

Nice To Haves

  • IV Certification highly preferred or must be obtained within 6 months.

Responsibilities

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.
