About The Position

Grey Stone Health & Rehab is hiring a Licensed Practical Nurse (LPN) for a full-time evening shift from 2 p.m. to 10 p.m. The role offers the opportunity to build meaningful relationships with residents and staff by learning their life stories and using nursing skills to support residents’ physical and psychosocial well-being. Nurses work within a collaborative care team and are supported by colleagues and the organization’s clinical standards, processes, and commitment to excellence. The position includes shift differential and experience pay.

Requirements

• Current, valid Licensed Practical Nurse license in the state of Indiana.
• Proficient medication management skills.
• Ability to conduct thorough assessments and accurately document changes in residents’ conditions.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Excellent communication and interpersonal skills.

Responsibilities

• Promote residents’ physical and psychosocial well-being through nursing care.
• Manage medications proficiently.
• Conduct thorough resident assessments and accurately document changes in residents’ conditions.
• Create a comforting and engaging atmosphere for residents.
• Promote teamwork within the care team to meet residents’ needs.
• Collaborate with nursing and other facility teams to provide coordinated, comprehensive care.
• Communicate effectively and support a respectful, positive work environment.
