About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Medical-Surgical Telemetry in Rochester, NY. The assignment is for a 14-bed temporary overflow med/surg telemetry unit designed to accommodate ED boarders, with flex space inside GI Endoscopy. The census may vary. The role involves providing direct patient care for adult medical-surgical patients with telemetry monitoring, maintaining telemetry observation, performing routine assessments, and responding to changes in patient status. Responsibilities include phlebotomy, IV placement, medication administration, wound care, JP/Hemovac care, Foley care, and NG/G-tube management. The LPN will also assist with oxygen delivery, EKG acquisition, vital signs, and isolation precautions, while supporting patient safety and comfort. Documentation in the EPIC charting system is required, including admission assessments and SBAR communications. Collaboration with an interdisciplinary team is essential for care coordination. Note that LPNs may have specific assignment limitations and may be utilized as a Patient Care Technician (PCT) as assigned.

Requirements

  • NY license required in-hand for submission; pending licenses not accepted
  • Certification: BLS required
  • Minimum 1 year RN experience required; previous charge experience preferred

Responsibilities

  • Provide direct patient care for adult medical-surgical patients with telemetry monitoring, under supervision as applicable
  • Maintain telemetry observation, perform routine assessments, and respond to changes in patient status per unit protocol
  • Perform phlebotomy and IV placement competency (mandatory) as part of routine care per unit needs
  • Administer medications per orders; manage wound care, dressing changes, JP/Hemovac care, Foley care, and NG/G-tube management
  • Assist with oxygen delivery, EKG acquisition, vital signs, and isolation precautions; support patient safety and comfort
  • Document in the EPIC charting system; complete admission assessments and SBAR communications for ED transfers and other admissions
  • Collaborate with interdisciplinary team (IV team, PT, Respiratory, Interpretation, Radiology, Social Services, Rapid Response, Pharmacy, Transport, Unit Secretary) to coordinate care
  • LPN role note: LPNs may not assume certain assignments or act as a resource in GIOF/med-surg; may be used as a patient care tech (PCT) as assigned
