About The Position

Forest Manor Healthcare Center is seeking a compassionate, dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care to residents at its skilled nursing and rehabilitation facility in Blairstown, New Jersey. This is a morning-shift, per-diem position. The role involves working with residents and collaborating with healthcare professionals to support comprehensive care.

Requirements

• Licensed Practical Nurse (LPN).
• At least 1 year of experience.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and record vital signs, including blood pressure, temperature, and pulse.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Provide wound care and maintain wound documentation.
• Assist residents with activities of daily living, including bathing, dressing, and grooming.
• Participate in quality improvement initiatives and maintain accurate records.
• Collaborate with other healthcare professionals to ensure comprehensive care.
