About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes coordinating with the RN and physician, supporting patients and caregivers with self-care, monitoring patient health, administering ordered specialized procedures, and completing timely, accurate documentation. The position offers performance recognition, clear direction, and development opportunities for other roles. Compensation includes per-visit-point pay for patient visits and hourly pay for certain non-visit activities, such as orientation. The advertised annual total cash target pay assumes full-time employment at 40 hours per week and full productivity; actual compensation may vary based on factors including local labor markets, education, work experience, productivity, and performance.
Requirements
- Current LPN license specific to the state or states assigned.
- Current CPR certification.
- At least 1 year of LPN or LVN experience.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by a physician, such as foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
