After Hours LPN Weekends (Fri/Sat/Sun 8p-8a)
About The Position
Care 360 Hospice is hiring an After Hours Licensed Practical Nurse to provide weekend night hospice nursing services in Kettering, Ohio, and the surrounding area. The full-time schedule is Friday, Saturday, and Sunday from 8 PM to 8 AM. The role includes scheduled skilled visits and after-hours on-call services for patients receiving end-of-life care in facilities and homes. The LPN responds to patient and family calls, assesses urgent symptoms through visits, assists with the plan of care and specialized procedures under RNCM direction and physician orders, supports patient and family education, and communicates changes to the RNCM. The position also involves collaboration with the nursing team, emergent visits including continuous and companion care as needed, and ongoing professional development.
Requirements
- Must be a Licensed Practical Nurse in the state of practice.
- At least 1 year of experience in skilled nursing.
- Valid driver's license and auto insurance coverage.
- Reliable transportation and willingness to travel.

Responsibilities
- Provide skilled nursing care to end-of-life patients during visits in facilities and homes.
- Provide after-hours on-call services and complete scheduled skilled visits.
- Respond to calls from patients and families concerning symptoms, changes in condition, or emergencies.
- Conduct home or facility visits to assess critical symptoms, including pain, respiratory distress, or emotional crisis.
- Assist the RNCM in carrying out the plan of care.
- Assist the nursing team with specialized procedures, as defined by the state nurse practice act, under RNCM direction and physician orders.
- Participate in IDGH collaboration to keep the patient plan of care current and address identified issues.
- Reinforce patient and family education regarding end-of-life care.
- Keep the RNCM informed of changes noted during care.
- Assist with emergent visits, including continuous care and companion care, as needed.
- Pursue professional growth and development through nursing organizations, in-service programs, on-the-job learning, further education, workshops, conferences, professional meetings, research, and reading.
- Perform other duties as assigned.
