About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role focuses on personalized care, supporting patients and caregivers, and coordinating with physicians and registered nurses to address patient needs. Compensation includes per-visit-point pay for visits and hourly pay for certain non-visit activities. Annual total cash compensation assumes full-time employment at 40 hours per week and full productivity.

Requirements

• Current LPN license specific to the state or states assigned for work.
• Current CPR certification.
• At least 1 year of LPN or LVN experience.
• Valid driver's license, reliable transportation, and liability insurance.
• Pass a drug test before beginning employment.

Responsibilities

• Verify the plan of care with the RN before each visit.
• Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by the physician, such as Foley catheter changes.
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation accurately and on time.
