About The Position

Masonicare Health Center in Wallingford, Connecticut is hiring a Licensed Practical Nurse for a day-shift position working 16 hours per week, every other weekend. The LPN provides nursing care to assigned patients under the supervision of an RN, using evidence-based nursing practices. Care includes medication and treatment administration, teaching, and implementing the interdisciplinary care plan. The role also contributes to care-plan development, collaborates with patients, families, colleagues, and the interdisciplinary team, maintains accurate documentation using the Electronic Medical Record, and supervises CNA care. The LPN participates in educational and quality-improvement activities, attends required annual in-services, and follows the Diligent Minimal Lift Program.

Requirements

Graduate of an accredited school of nursing for practical nursing.
Current State of Connecticut license to practice as a licensed practical nurse.

Nice To Haves

At least 1 year of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

Provide nursing care appropriate to residents’ or patients’ ages, needs, and abilities, including medication and treatment administration, teaching, and implementation of the interdisciplinary care plan.
Contribute to care-plan development by collecting and recording data on an ongoing and systematic basis.
Communicate and collaborate with residents or patients, families, colleagues, and interdisciplinary team members to meet patient and unit needs.
Document accurate, pertinent, and comprehensive information in a timely and concise manner, in accordance with regulations and facility policies and procedures; competently use the Electronic Medical Record.
Supervise CNA care to ensure compliance with the interdisciplinary care plan and resident profile.
Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge; attend all annual mandatory in-services.
Participate in quality-improvement programs as directed.
Follow standards of the Diligent Minimal Lift Program.
Perform other duties as required.
