About The Position

Horizons Residential Care Center is seeking a Licensed Practical Nurse to provide relationship-focused nursing care to children and adults with intellectual and developmental disabilities in an Intermediate Care Facility (ICF) setting. Nurses work as part of a close-knit care team, building ongoing relationships with residents and supporting their quality of life through holistic, person-centered care. This full-time role is based in Rural Hall, North Carolina, with a 7:00 AM–7:00 PM schedule. Horizons describes its work environment as smaller and relationship-focused, with consistent residents, supportive leadership, predictable routines, and a mission-driven nonprofit setting. Nurses collaborate with physicians, therapists, QIDPs, Direct Support Professionals, Medication Technicians, and other interdisciplinary team members.

Requirements

Active North Carolina LPN License.
Strong communication and documentation skills.
Experience with electronic health records.
Valid North Carolina Driver's License.

Nice To Haves

Experience in long-term care, residential care, developmental disabilities, pediatrics, or ICF settings.
CPR/BLS Certification (Adult & Pediatric).

Responsibilities

Provide direct nursing care in an Intermediate Care Facility (ICF) setting.
Administer and document medications according to physician orders.
Monitor resident health status and communicate changes appropriately.
Collaborate with physicians, therapists, QIDPs, and interdisciplinary team members.
Maintain accurate documentation within electronic health record systems.
Complete medication counts and monitor medication inventory.
Provide clinical guidance and support to Direct Support Professionals and Medication Technicians.
Participate in care planning and resident advocacy.
Respond appropriately to medical emergencies and resident needs.
Promote resident dignity, independence, and overall well-being.
