About The Position

The Licensed Practical/Vocational Nurse provides direct patient care under the supervision of a registered nurse. The role follows the plan of care, provides treatments, and collaborates with team members to support positive patient care outcomes. The position also participates in on-call duties and may be assigned other job-related tasks.

Requirements

• Graduate of an accredited practical nurse or vocational nursing program.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Comply with accepted professional standards and practice.
• Demonstrate good verbal and written communication and organization skills.
• Be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
• Possess and maintain current CPR certification.

Nice To Haves

• Three years of nursing experience preferred.
• Community/home health or medical/surgical experience preferred.

Responsibilities

• Provide direct patient care as defined by the applicable state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patients and family members or caregivers with education and information relevant to the diagnosis and plan of care.
• Coordinate home health services and report identified needs for other disciplines, including HHA, OT, PT, MSW, ST, and Dietician, to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
• Use equipment and supplies effectively and efficiently.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
• Participate in on-call duties as defined by the on-call policy.
