About The Position

River Valley Home Health and Hospice is hiring a Licensed Practical Nurse to provide home health services in the Colorado River Valley. The nurse provides direct patient care within the State’s Nurse Practice Act and implements the plan of care initiated by a registered nurse. The role includes documenting care, gathering and reporting assessment information, coordinating with the care team, and using equipment and supplies effectively. The position requires travel for agency-related activities and patient care.

Requirements

Complete a requisite practical (vocational) nursing program or be licensed in the state where the home health agency is located.
Hold a current Licensed Practical Nurse or Licensed Vocational Nurse license in the state.
Comply with accepted professional standards and practice.
Demonstrate good verbal and written communication and organization skills.
Be able to drive and/or have access to transportation for agency-related travel.
Maintain a valid driver’s license and automobile insurance coverage, and have access to an automobile.
Possess and maintain current CPR certification.

Nice To Haves

Three years of nursing experience preferred.
Community/home health or medical-surgical experience preferred.

Responsibilities

Provide direct patient care as defined in the State’s Nurse Practice Act.
Implement the plan of care initiated by the registered nurse.
Document care accurately and promptly in accordance with the plan of care.
Collect assessment data and make observations, reporting findings to and collaborating with registered nurses and physicians.
Participate in coordinating home health services and report identified needs for other disciplines to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
Use equipment and supplies effectively and efficiently.
Participate in personal and professional growth and development.
Perform other duties as assigned by the registered nurse.
Follow written and oral instructions.
Maintain patience, tact, a cheerful disposition, and enthusiasm.
Comply with all training requirements.
