About The Position
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care, as prescribed by a physician, to acutely or chronically ill patients in their homes. The nurse records clinical and progress notes and coordinates patient care activities and services to provide high-quality care in the assigned geographic service area. The role works under the direct supervision of a Registered Nurse and/or designated supervisor. PRN positions are available in the Clarksville area.
Requirements
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or access to public transportation.
Nice To Haves
- Current CPR certification or ability to complete certification within 90 days of hire.
- Proven ability to manage multiple tasks simultaneously.
- Proven ability to work independently.
- Excellent communication, writing, and organizational skills.

Responsibilities
- Provide high-quality clinical services within the scope of practice defined by state nursing laws, in accordance with the plan of care and in coordination with other health care team members, following applicable infection control procedures.
- Document patient visits thoroughly and completely in the electronic medical record according to regulatory and payer requirements, using the agency’s electronic visit verification program as applicable.
- Participate in the agency’s care coordination process by promptly reporting patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, clinicians, agency management staff, case coordinators, and community case managers as applicable.
- Provide care in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
