About The Position

CLFC Healthcare and Communications is hiring a per diem Licensed Practical Nurse to work on-site at Wayne County’s 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has an RN on every shift. The LPN runs medications and treatments on an assigned unit and works with residents, CNAs, and the charge RN. This W-2 hourly role offers a self-selected schedule with no minimum hours. Nurses choose available shifts through the BRIDGE App; shifts are typically eight paid hours with a half-hour unpaid meal break, and twelve-hour shifts may be available. The stated shift times are 6:30 AM–3:00 PM, 2:30 PM–11:00 PM, and 10:30 PM–7:00 AM. The position pays $38–$42 per hour, with nights and weekends paid at the top of the range, and overtime over 40 hours and all ten County holidays paid at time and a half. Pay is weekly, and orientation is paid at the nurse’s rate. CLFC reimburses New York license and CPR renewals, supports continuing education and certifications, and offers up to $1,500 per year in tuition assistance toward an RN. A $500 referral bonus is available for referring a nurse or aide. Fingerprinting, immunizations, screening, and license verification are arranged and paid by CLFC. Paid sick leave accrues under New York law, and CLFC professional liability insurance covers each shift. The Program Manager assists with scheduling and pay questions. A confirmed shift canceled by the facility two hours or less before its start is paid for four hours; cancellations with more notice are removed from the schedule.

Requirements

Hold a current, unrestricted New York State Licensed Practical Nurse license.
Graduate from an approved practical nursing program.
Hold current BLS (CPR).
Have one year of recent LPN experience.
Be able to complete the New York State Department of Health Criminal History Record Check and a drug screen.
Be current on immunizations required by New York for nursing home personnel.

Responsibilities

Run the medication pass and deliver treatments according to physician orders and facility policy.
Observe, document, and report changes in resident condition to the charge RN.
Support and direct CNAs on the unit within the New York scope of practice.
Keep shift documentation current in the facility’s system.
Follow facility infection-control, safety, and sign-in practices.
