About The Position

CLFC Healthcare and Communications is hiring a per diem Licensed Practical Nurse for Wayne County's 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has an RN on every shift. The LPN will provide resident care on an assigned unit, with on-site leadership and support from the charge RN. This is a W-2 hourly per diem role with self-directed scheduling and no minimum hours. Nurses select available shifts through the BRIDGE App. Shifts include 6:30 AM–3:00 PM, 2:30 PM–11:00 PM, and 10:30 PM–7:00 AM, with eight paid hours and a half-hour unpaid meal break; twelve-hour shifts may also be available. The position may be a fit for nurses located in Wayne, Ontario, Seneca, or Monroe County. Pay is $38.00–$42.00 per hour, determined by experience, with night and weekend shifts paid at the top of the range. Overtime after 40 hours and all ten County holidays are paid at time and a half. Pay is weekly, and orientation is paid at the employee's rate. A confirmed shift canceled by the facility two hours or less before it begins is paid for four hours; with more notice, the shift is removed from the schedule. CLFC reimburses New York license and CPR renewals, supports continuing education and certifications, and offers up to $1,500 per year in tuition assistance toward an RN. A $500 referral bonus is available for referring a nurse or aide to the roster. Fingerprinting, immunizations, screening, and license verification are arranged and paid by CLFC. Paid sick leave accrues under New York law, and professional liability insurance covers every shift. The role is supported by a Program Manager and is part of CLFC's broader staffing roster.
