About The Position
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) for a night shift Long-Term Care assignment. The role involves providing direct nursing care to residents in a Skilled Nursing Facility while ensuring patient safety, comfort, and high-quality care.
Requirements
- Active Colorado LPN License
- Strong clinical and communication skills
- Ability to work night shifts and adapt to scheduling needs
Nice To Haves
- Experience in LTC, SNF, or post-acute care settings
- BLS Certification
Responsibilities
- Provide direct patient care under RN or physician supervision
- Administer medications and treatments as prescribed
- Monitor resident conditions and report any changes
- Assist with admissions, discharges, and assessments
- Maintain accurate documentation and charting
- Collaborate with interdisciplinary healthcare teams
- Ensure compliance with safety and infection control standards
- Support residents with daily care needs while maintaining dignity
