About The Position
We are seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality care in a Long Term Care (LTC) setting. The ideal candidate will be experienced in supporting elderly residents, administering medications, and collaborating with healthcare teams to ensure patient safety and comfort.
Requirements
- Active Wisconsin LPN License
- Current CPR/BLS Certification
- Prior experience in Long Term Care (LTC) or nursing preferred
- Strong attention to detail and communication skills
Responsibilities
- Administer medications safely following the “5 Rights” of medication administration
- Monitor residents’ health conditions and promptly report any changes
- Communicate effectively with physicians, charge nurses, and family members
- Support CNAs and respond to resident call lights in a timely manner
- Report any suspected abuse, neglect, or misappropriation
- Assist with daily resident care and maintain accurate documentation
