About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The role is part of a team that supports residents and associates and builds lasting relationships. The schedule is Wednesday and Thursday from 3 p.m. to 11 p.m., and Friday plus every other weekend from 7 a.m. to 7 p.m.

Requirements

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program, as required by the state.
• One to two years of nursing experience preferred.
• Previous experience working with older adults in a paid or volunteer position is highly desirable.

• Previous experience working with the elderly in a paid or volunteer position.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, in accordance with state regulations.
• Direct and supervise Resident Care Assistants’ daily work assignments.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with supporting resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report it to appropriate sources in accordance with state regulations.
• Respond to emergencies and personal emergency response systems, and know when to call for backup.
