About The Position
Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join the team at Putnam Farms. The LPN delivers nursing care to residents in a warm, comfortable, home-like environment and works with a supportive team to build community and lasting relationships with associates and residents. The advertised full-time schedule is evenings, 3 p.m.–11 p.m.; part-time evening and weekend opportunities may also be available.
Requirements
- Current state LPN license.
- CPR certification.
- Graduate of an approved LPN program, as required by the state.
- One to two years of nursing experience is preferred.
- Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities
- Accurately assess, communicate, and document residents’ status.
- Observe and report significant changes in residents’ behavior or health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, as required by state regulations.
- Direct and supervise Resident Care Assistants’ daily work assignments.
- Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
- Coordinate care needs with community providers through an effective case management process.
- Balance service demands with supporting residents’ independence.
- Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources as required by state regulations.
- Respond to emergencies and personal emergency response system alerts, and determine when to call for backup.
