About The Position

Green Oaks of Goshen is hiring a Licensed Practical Nurse (LPN) for a PRN, part-time position in Goshen, Indiana, on the 2nd shift. The role pays $28.00–$30.00 per hour. The LPN oversees medication distribution in accordance with physician orders and the community’s medication management procedures, and supports resident safety, independence, and dignity through assessment and individualized service planning. The nurse also communicates with physicians, healthcare providers, and families about residents’ physical, mental, and social well-being, contributes to quality assurance and performance improvement, and participates in ongoing learning activities. Requirements

Current state licensure as a Licensed Practical Nurse (LPN).
Current CPR certification.
Exceptional communication and interpersonal skills.
Ability to serve with love, compassion, and dignity.

Responsibilities

Oversee medication distribution in accordance with physician orders and community medication management documentation and procedures, with a focus on resident safety.
Assess residents and create personalized service plans tailored to their care needs while supporting their independence and dignity.
Facilitate communication among physicians, healthcare providers, and families regarding residents’ physical, mental, and social well-being.
Contribute to quality assurance and performance improvement initiatives.
Attend and participate in educational seminars, staff meetings, and other learning experiences.
