About The Position

BAYADA Home Health Care is hiring Licensed Practical Nurses for its Transition Team in Philadelphia, Pennsylvania. The team supports transitions from hospital care or BAYADA’s HICU® Health Services team to ongoing home care. Nurses provide care to infants, children, and adults with complex and rare diseases, and deliver extensive training to the ongoing care team. The role involves partnering with CHOP and working in a teaching setting to prepare and educate the continuing care team. Openings are for overnight shifts, with availability needed for a consistent ongoing schedule. The position requires travel throughout Philadelphia County. Full-time or part-time scheduling is offered. Compensation is hourly and based on qualifications and experience; the posting lists Transition Team rates of $40–$42 per hour and standard rates of $30–$35 per hour.

Requirements

• Current Licensed Practical Nurse (LPN) license.
• NICU, PICU, ICU, or pediatric home care trach/vent experience.
• Passing score on high-tech adult and pediatric simulations.
• Current CPR certification.
• Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team.
• Availability to commit to an ongoing, consistent schedule.
• Ability to travel throughout Philadelphia County.
