About The Position
Sentara Home Care Services in Charlottesville is hiring a Licensed Practical Nurse (LPN) to provide direct, one-on-one care to patients in their homes. The role includes monitoring patient health, administering treatments, managing medications, and supporting patients with a range of medical needs. The LPN collaborates with Registered Nurses and physicians to provide compassionate, high-quality care that improves patient well-being. This is a full-time, first (day) shift position based in Charlottesville, Virginia, with 100% travel in a personal vehicle. Mileage reimbursement is provided for travel between patient visits. The position offers a base pay range of $28.57–$43.04 per hour for full-time employment. Additional compensation may include shift differentials, on-call pay, overtime, shift premiums, extra-shift incentives, or bonuses. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
Requirements
- Trade school graduate.
- Active Licensed Practical Nurse (LPN) license in Virginia.
- Basic Life Support (BLS) certification required within 90 days.
- At least 2 years of LPN experience.
- Personal vehicle for travel between patient visits.
Responsibilities
- Serve as a patient advocate in the prevention of illness or disease.
- Provide care and treatment to patients, along with clinical support and teaching for patients and family members.
- Provide customer service, testing, medication administration, and implementation of nursing interventions and care plans.
- Support a holistic approach to patient care.
- Monitor patient health, administer treatments, and manage medications in patients’ homes.
- Collaborate with Registered Nurses and physicians to deliver high-quality, compassionate care.
