About The Position

Cumberland Trace is hiring full-time Licensed Practical Nurses for all shifts and weekend option shifts. All shifts require working every third weekend. At CarDon and Associates, LPNs provide high-quality patient care, support residents’ daily activities, and help ensure their comfort and well-being. They collaborate with a multidisciplinary team to develop and implement individualized care plans, monitor residents’ health, administer medications, and educate residents and their families. Requirements

Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
Basic Life Support (BLS) certification.
Work every third weekend.

Nice To Haves

Experience in a long-term care or assisted living environment.
Proficiency in electronic health record (EHR) systems.

Responsibilities

Administer medications and treatments as prescribed by the healthcare provider.
Monitor and record vital signs and report changes in residents’ conditions to the nursing staff.
Assist residents with daily living activities, including bathing, dressing, and grooming.
Maintain accurate and up-to-date resident records and documentation.
Collaborate with healthcare team members to develop and implement care plans.
Provide education to residents and their families.
Support residents’ comfort, well-being, quality of life, and health goals.
