About The Position

Wayne County’s county-operated, 192-bed skilled nursing and rehabilitation facility in Lyons, New York, is hiring per diem LPNs. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has an RN on every shift. The role involves providing medication passes and treatments on an assigned unit, with on-site leadership and RN support. This is a W-2 hourly per diem position with a schedule selected by the nurse through the BRIDGE App. There are no minimum hours; nurses may choose available shifts, including eight-hour shifts with a half-hour unpaid meal break or twelve-hour shifts when available. Shifts may be any day of the week. Compensation is $38.00–$42.00 per hour, with experience determining the rate and nights and weekends paid at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. Pay is weekly, and orientation is paid at the nurse’s rate. The employer reimburses New York license renewal and CPR renewal, supports continuing education and certifications, and offers up to $1,500 per year in tuition assistance toward an RN. A $500 referral bonus is offered for bringing a nurse or aide onto the roster. Fingerprinting, immunizations, screening, and license verification are arranged and paid for by CLFC. Paid sick leave accrues under New York law, and professional liability insurance covers each shift. If the facility cancels a confirmed shift two hours or less before it starts, the nurse is paid for four hours; with more notice, the shift is removed from the schedule. CLFC Healthcare and Communications, operated by CLF Consultants LLC, provides healthcare staffing and language access services to federal, state, and local agencies. The company describes a nationwide network of more than 500 healthcare professionals and more than 200 interpreters. It is an equal opportunity employer.
