Requirements
- Completion of an accredited practical (vocational) nursing program or licensed in the state where the agency operates
- Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
- Complies with accepted professional standards and practice
- Demonstrates strong verbal and written communication skills
- Strong organizational skills
- Must be a licensed driver with an insured and reliable automobile
- Maintains current CPR certification
Nice To Haves
- Three years nursing experience preferred
- Community/home health or medical-surgical experience preferred
Responsibilities
- Provides direct patient care as defined in the State’s Nurse Practice Act
- Implements plan of care initiated by the registered nurse
- Provides accurate and timely documentation consistent with the plan of care
- Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s)
- Participates in coordination of home health services, appropriately reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Director of Nursing
- Uses equipment and supplies effectively and efficiently
- Participates in personal and professional growth and development
- Performs other duties as assigned by the registered nurse
