About The Position

Gallagher Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join their hospice team. This role offers the opportunity to make a meaningful impact by providing comfort, dignity, and support to patients and families during end-of-life care. The position involves providing hands-on patient care, following care plans, educating patients and families, collaborating with an interdisciplinary team, and managing pain and symptoms.

Requirements

• Graduate of an accredited nursing program
• At least 1 year of nursing experience
• Active LPN license
• Strong communication and organizational skills
• Valid driver’s license and reliable transportation

• Hospice, home health, or med/surg experience is a plus

Responsibilities

• Provide compassionate, hands-on patient care, including end-of-life support
• Follow individualized care plans and keep documentation accurate and timely
• Educate patients and families so they feel supported and informed
