About The Position
Avita Health System is seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Health & Hospice Department in Galion, Ohio. The Home Health and Hospice Licensed Practical Nurse provides quality and safe delivery of home health care services under the guidance of the HHH Registered Nurse. Assists in the provision of home health care services that reflect the home care agency's philosophy and standards of home health and nursing care of assigned clients.
Requirements
- State of Ohio Licensed Practical Nurse licensure.
- BLS certification or obtained within 30 days of hire.
- Current valid Ohio driver’s license.
Responsibilities
- Provides quality and safe delivery of home health care services under the guidance of the HHH Registered Nurse.
- Assists in the provision of home health care services that reflect the home care agency's philosophy and standards of home health and nursing care of assigned clients.
