About The Position

Gallagher Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join their hospice team. This role focuses on providing comfort, dignity, and support to patients and families during end-of-life care. The LPN will perform home hospice visits, ensuring patients receive meaningful care and families feel supported and informed.

Requirements

• Graduate of an accredited nursing program
• At least 1 year of nursing experience (hospice, home health, or med/surg is a plus)
• Active LPN license
• Strong communication and organizational skills
• Valid driver’s license and reliable transportation

Responsibilities

• Provide compassionate, hands-on patient care, including end-of-life support
• Follow individualized care plans and keep documentation accurate and timely

• Manage pain and symptoms to ensure comfort and dignity
