About The Position

Southeast Colorado Hospital District is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join its Long Term Care team for a 13-week contract assignment. The LPN is responsible for providing direct and indirect nursing care to residents while ensuring high-quality, patient-centered care in a collaborative environment. This role includes supervising Certified Nursing Assistants (CNAs), administering medications, coordinating resident care, and maintaining accurate clinical documentation.

Requirements

  • Active Licensed Practical Nurse (LPN) license.
  • Current BLS (Basic Life Support) certification.
  • Minimum of one year of nursing experience.
  • Strong clinical assessment, communication, and organizational skills.

Nice To Haves

  • Long Term Care experience is preferred.

Responsibilities

  • Provide comprehensive nursing care to residents during assigned shifts.
  • Assign, direct, and supervise resident care provided by CNAs and support staff.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor residents' conditions and promptly report significant changes to nursing leadership.
  • Document resident assessments, acute illnesses, physician visits, passes, and other clinical updates accurately and in a timely manner.
  • Communicate resident status, care updates, and new physician orders with families and designated medical power of attorney (MDPOA) as appropriate.
  • Collaborate with physicians, nursing leadership, and interdisciplinary teams to ensure continuity of care.
  • Promote teamwork and effective communication across departments and shifts.
  • Participate in staff meetings, education sessions, and ongoing training.
  • Perform additional duties as assigned to support quality resident care.
