About The Position

The primary function of the hospice Licensed Practical Nurse (LPN) in the Shenandoah House is to provide nursing care under the supervision of a registered nurse for patients of all ages and coordinate care with the hospice IDT, patient, and family. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served at the Shenandoah House. Must be able to provide the care needed as described in the department's policies and procedures. This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities.

Requirements

  • Graduate of an Accredited School of Vocational/Practical Nursing
  • Current Commonwealth of Virginia license as a Licensed Vocational/Practical Nurse, or a multi-state license
  • CPR- Basic Life Support Healthcare Provider certification (BLS HCP)
  • Medical/Surgical experience in hospital or clinical setting within the last year.
  • Driver’s License: Yes
  • To drive an Augusta Health Vehicle, must also be 21 years old and Motor Vehicle (MVR) must be approved by Risk Management.
  • Eligibility to work in the United States and meet Virginia state employment requirements
  • Ability to establish priorities, work independently, and proceed with objectives.
  • Ability to meet consistent and punctual attendance.
  • Must have excellent written and verbal communication skills.
  • Ability to interact with colleagues, supervisors and customers face to face.
  • Ability to be flexible.
  • Demonstrates job related computer competence as seen in patient documentation and information retrieval. Documentation is submitted timely.
  • Skill to use a personal computer and various software packages.
  • Ability to remain calm under stressful situations

Responsibilities

  • Practices within the VA Nurse Practice Act and LPN scope of practice.
  • Ensures compliance with HIPAA regulations to maintain patient confidentiality.
  • Demonstrates respect for patient dignity, with no reported breaches or complaints from patients or fellow care providers.
  • Performs data collection and nursing interventions safely, following physician orders.
  • Continuously reassesses patients and promptly reports changes in condition to the RN.
  • Provides patient teaching and follow-up care as appropriate.
  • Supports patients in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
  • Involves family members in care and patient education.
  • Maintains a clean, safe, and supportive environment.
  • Prepares light meals as needed.
  • Coordinates patient care with other healthcare providers and interdisciplinary team (IDT) members, ensuring accurate handoff communication.
  • Consistently follows CDC hand hygiene guidelines and utilizes personal protective equipment (PPE) appropriately.
  • Demonstrates knowledge of safe patient handling techniques and equipment.
  • Identifies fall risks and implements preventive measures.
  • Uses patient two-identifier verification consistently.
  • Appropriately labels specimens at the bedside.
  • Ensures 100% compliance with the Five Rights of Medication Administration.
  • Demonstrates knowledge of all prescribed medications, including high-risk drugs.
  • Maintains secure medication storage and handling.
  • Assesses pain relief after medication administration and promptly reports unrelieved pain to the RN.
  • Participates in quality improvement projects and staff satisfaction initiatives.
  • Contributes to the Quality Assurance and Performance Improvement (QAPI) plan.
  • Provides input on cost containment and resource management.
  • Maintains readiness for surveys and ensures compliance with environmental and policy standards.
  • Uses safety devices, shields, and proper body mechanics to minimize injury risks.
  • Disposes of sharps safely and correctly.
  • Completes all annual education and department competency requirements.
  • Takes responsibility for ongoing professional development and learning.
  • Maintains a positive attitude and adaptability within the team.
  • Demonstrates reliable attendance and minimizes scheduling disruptions.
  • Attends staff meetings and in-service training sessions as required.
  • Supports the hospice philosophy and delivery of compassionate end-of-life care.
  • Acts as a mentor to new staff and students when needed.
  • Demonstrates collaboration and cooperation with the interdisciplinary team to meet patient needs
