About The Position

The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to the patients, taking vital signs, carrying out screening duties, and administering medications. This role requires supervision in accordance with current Federal, State, and local standards, guidelines, and regulations. The LPN demonstrates an understanding of and embraces the mission statement of Christian Health. Ramapo Ridge Behavioral Health (RRBH) is a 58-bed inpatient psychiatric hospital serving adults from 18 years of age through the senior years. It includes a general adult psychiatric division and a specialized geriatric division for dementia and Alzheimer's care. Haven at Ramapo Ridge provides inpatient trauma-informed care for women only, offering personalized care plans in a safe space. RRBH has received recognition for its innovative Haven program.

Requirements

  • Must be a graduate of an accredited school of nursing.
  • Demonstrates compliance with the CH ASPIRE Standards of Performance.
  • The individual is competent to care for adult and geriatric (ages 65+) patients.

Nice To Haves

  • Previous experience in long term care and/or geropsychiatric setting preferred.
  • One year experience with geriatric population preferred.

Responsibilities

  • Directs the day-to-day functions of the PNAs in accordance with current rules, regulations and guidelines that govern the facility.
  • Signs and dates all entries made in the patient's medical record.
  • Makes written and oral reports/recommendations concerning the activities of shift as required.
  • Charts nurses' notes in an informative and descriptive manner that reflects the care provided to the patient as well as the response to the care.
  • Develops work assignments and/or assists in completing and performing such tasks.
  • Prepares and administers medications as ordered by the physician and within the guidelines of good nursing practice.
  • Ensures that discontinued drugs and narcotics are turned in to the Nursing Supervisor/Unit Manager for destruction or returned to provider pharmacy for credit.
  • Reviews medication orders for completeness of information, accuracy in the transcription of the physician's order, and adherence to stop policies.
  • Makes daily rounds of assigned unit to ensure that the nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
  • Records new/changed diet orders and forwards information to the Dietary Department or Unit Clerk.
  • Transcribes physician's orders/LP orders, as per policy.
  • Reports all discrepancies noted concerning physician's orders, diet change, charting error, etc. to the Nursing Supervisor/Nurse Manager.
  • Fills out and completes Transfer Forms in accordance with established procedures.
  • Ensures that assigned PNAs are aware of the patient care plans and that they are used in administering daily care to the patient.
  • Completes all documentation as required.
  • Participates in the maintenance and implementation of the facility's quality assurance program for the nursing services department.
  • Interprets the department's policies and procedures to personnel, patients, visitors and government agencies as required.
  • Admits, transfers, and discharges patients as required.
  • Fills out and completes accident/incident reports and submits them to the Unit Manager as required.
  • Charts all accidents/incidents involving the patients following established procedures.
  • Takes and records TPR's, blood pressures, etc. as necessary.
  • Participates in the implementation and ensures maintenance of the infection control program for monitoring communicable and/or infectious diseases among the patients and personnel (e.g., universal precautions guidelines).
  • Monitors nursing care to ensure that all patients are treated fairly and with kindness, dignity, respect, and privacy.
  • Ensures that assigned nursing personnel follow established departmental policies and procedures including dress code and safety regulations.
  • Reviews patient care plans for target goals and individualized interventions, problems, approaches, and revisions based on nursing needs.
  • Informs the Nursing Supervisor/Unit Manager of any changes that need to be made on the care plan.
  • Ensures that nurses' notes reflect that the care plan is being followed.
  • Participates in implementation and maintenance of the procedures for reporting hazardous conditions or equipment.
  • Orders prescribed medications, supplies, and equipment as necessary and in accordance with established policies.
  • Ensures that an adequate supply of medications, supplies, and equipment are on hand to meet the nursing needs of the patients.
  • Reports needs to the Nursing Supervisor/Unit Manager.
  • Ensures that department personnel, patients, and visitors follow the department established policies and procedures at all times.
  • Provides leadership to nursing personnel assigned to unit/shift.
  • Creates and maintains an atmosphere of warmth, personal interest and positive emphasis as well as a calm environment throughout the unit and shift.
  • Gives/receives the nursing report upon reporting in and ending shift duty hours.
