Our OMH Adult Outpatient Clinic provides a multi-disciplinary team of licensed independent practitioners and clinicians to provide services for adults experiencing co-occurring symptoms and disorders. This collaborative approach to care, integrating medication with therapy, ensures the best outcomes for clients. In addition to helping individuals reduce symptoms, the focus of treatment is on developing self-awareness and identifying personal goals and strategies to achieve those goals. Our clinic uses evidence-based best practices in all treatment modalities, and takes a holistic approach to the individual.

Requirements

  • Licensed Practical Nurse (LPN) certification required, with current licensure in New York State
  • Demonstrated ability to work as part of a multi-disciplinary team
  • Able to sit for long periods of time
  • Able to use computer
  • Able to lift a minimum of 20 pounds

Nice To Haves

  • Minimum of one year experience providing nursing care to adults with severe and persistent mental illnesses preferred
  • Experience in a Community Health setting

Responsibilities

  • Provide and document injections, perform inventory and order supplies for client’s receiving injections
  • Maintain medication storage room and maintain inventory in a neat and orderly manner and in compliance with all applicable regulations
  • Assist psychiatrists and nurse practitioners in providing timely responses to client requests for prescription refills
  • Review client blood work and lab reports and inform treating psychiatrists and nurse practitioners of emergent issues and follow up as appropriate
  • Assist medical staff in completion of insurance prior-authorization forms
  • Assist clients/staff in crisis or medical emergency situations
  • Collaborate with service providers and other stakeholders as appropriate
  • Provide group and individual wellness education and perform general health assessments as needed
  • Exercise good professional and ethical judgment
  • Maintain appropriate professional boundaries
  • Demonstrate reliability and conscientiousness
  • Strong communication and team building skills
  • Good organization and time management skills
  • Strong verbal and written communication skills
  • Assist providing injections in PROS when needed
  • Other duties as assigned
