About The Position

The Licensed Practical Nurse oversees medication distribution according to physician orders and follows the community’s medication management documentation and procedures, with a strong focus on resident safety. After assessing residents, the LPN creates personalized service plans that address individual care needs while supporting resident independence and dignity. The role also helps coordinate communication among physicians, healthcare providers, residents’ families, and others regarding residents’ physical, mental, and social well-being. The LPN contributes to quality assurance and performance improvement initiatives and participates in ongoing learning and development activities.

Requirements

• Current state licensure as a Licensed Practical Nurse (LPN).
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Ability to serve with love, compassion, and dignity.
