About The Position

As a CenterWell Home Health Licensed Practical Nurse, you provide personalized, one-on-one skilled care to patients in their homes in the DeKalb and Aurora areas. Reporting to the Clinical Manager and working under the direction of an RN, you collaborate with physicians, clinicians, patients, and families to support recovery, independence, and individualized care plans. The role includes patient and family education, accurate clinical documentation, communication about patient needs and status changes, care coordination, and discharge planning.

Requirements

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families about disease management, treatment options, and self-care techniques.
• Document patient visits, interventions, and outcomes accurately.
• Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
• Participate in care coordination activities and discharge planning.
