About The Position
Navitas Healthcare, LLC is seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a clinic setting. The LPN will provide patient-centered care, support clinical assessments, and collaborate with the healthcare team to deliver high-quality outpatient services.
Requirements
- Active Minnesota LPN License (required at submission)
- Minimum 2 years of LPN experience, preferably in clinic or outpatient settings
- Experience with EPIC EMR preferred
- Strong knowledge of clinical procedures and patient care practices
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced, team-oriented environment
- Strong organizational skills and attention to detail
Responsibilities
- Perform and document patient assessments within LPN scope of practice
- Assist with patient health histories and admission processes
- Identify patient care needs and support care planning with the clinical team
- Implement and monitor care plans under the direction of licensed practitioners
- Observe, report, and respond to changes in patient condition
- Administer medications, treatments, and nursing interventions safely and accurately
- Maintain timely and accurate clinical documentation
- Support discharge planning and patient education initiatives
- Communicate effectively with patients, families, and interdisciplinary teams
- Ensure proper task delegation while maintaining patient safety standards
- Follow all facility policies, procedures, and clinical guidelines
