About The Position

We are seeking a full-time Licensed Practical Nurse (LPN), Behavioral Health to join our Assertive Community Treatment (ACT) team and provide compassionate, community‑based care to adults with significant mental health and substance use needs. Requirements

• Active, unrestricted LPN license in North Carolina
• Ability to work in community settings
• Valid driver’s license, vehicle insurance, and reliable transportation
• Strong communication, documentation, and relationship-building skills

Nice To Haves

• Experience in mental health or substance use services

Responsibilities

• Administer psychotropic medications and monitor symptoms and health status.
• Document all care and observations using the Welligent EMR platform.
• Collaborate with the multidisciplinary ACT team in developing individualized treatment plans.
• Participate in daily team meetings to ensure coordinated, client‑centered support.
• Provide crisis intervention services as part of a rotational on‑call schedule.
• Deliver nursing care in homes and community settings.
• Advocate for client choice, educate on wellness and health management, and connect individuals with needed resources.
