About The Position
EmpowerMe Wellness is hiring a PRN Licensed Physical Therapist to provide therapy and wellness services for older adults in senior living communities. The therapist is part of EmpowerMe 360, an integrated care model that connects therapy, wellness, and medical providers to support residents in living stronger, safer, and more independent lives. The role combines clinical therapy with care coordination and wellness support, helping residents maintain independence, confidence, and satisfaction. The position offers predictable schedules with no late nights and no constant driving. Therapists work collaboratively with residents, caregivers, and other providers, and may contribute to program development and quality improvement. EmpowerMe Wellness is an integrated healthcare company partnering with senior living communities nationwide.
Requirements
- Hold a current professional license in the relevant therapy discipline; the posting lists PT, PTA, OT, COTA, or SLP.
- Have a passion for empowering seniors to live their best lives.
- Demonstrate strong clinical skills and a collaborative spirit.

Responsibilities
- Provide therapy and wellness services for older adults.
- Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
- Support care coordination through wellness check-ins, caregiver communication, and telehealth support.
- Maintain professional and ethical standards in line with company initiatives.
- Contribute to program development and continuous quality improvement.
- Help residents maximize independence, confidence, and satisfaction.
