About The Position
EmpowerMe Wellness is seeking a licensed Physical Therapist to provide therapy and wellness services to older adults in senior living communities. The role is part of EmpowerMe 360, an integrated care model connecting therapy, wellness, and medical providers. The therapist helps residents live stronger, safer, and more independently through assessments, personalized care plans, evidence-based interventions, care coordination, and wellness support. The position combines direct therapy with wellness check-ins, caregiver communication, and telehealth support. It offers predictable scheduling without late nights or constant driving, and opportunities for leadership, specialty programs, professional development, and cross-training. Therapists work collaboratively and contribute to program development and continuous quality improvement.

Responsibilities
