Licensed Physical Therapist (PT) PRN
About The Position
EmpowerMe Wellness is seeking a licensed physical therapist to provide therapy and wellness services for older adults through its EmpowerMe 360 integrated care model. The role helps senior residents live stronger, safer, and more independent lives by delivering assessments, personalized care plans, and evidence-based interventions as part of a collaborative team. The position also supports care coordination through wellness check-ins, caregiver communication, and telehealth support. The therapist maintains professional and ethical standards, contributes to program development and continuous quality improvement, and helps residents maximize independence, confidence, and satisfaction. Compensation is $55.00–$65.00 per hour.
Requirements
- Current professional license in the applicable therapy discipline; the posting lists PT, PTA, OT, COTA, or SLP.
