About The Position

The role supports residents in living stronger, safer, and more independent lives through therapy, wellness, and coordinated care. The position includes delivering evidence-based interventions, contributing to personalized care plans, and collaborating with wellness and medical providers. It also involves care coordination, wellness check-ins, caregiver communication, and telehealth support. The role offers predictable scheduling without late nights or constant driving, along with opportunities for professional development and leadership.

- Current professional license in the relevant therapy discipline, including PTA.

Responsibilities
