About The Position

Brookhaven Hospice is seeking a full-time Hospice Aide (LNA) to provide hands-on care that brings comfort and dignity to patients at the end of life. Working closely with the nursing team, the LNA helps meet patients’ physical, emotional, and personal needs with respect, empathy, and professionalism. The position covers Bedford, Hooksett, Concord, and Manchester, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. Brookhaven Hospice describes itself as a mission-driven, patient-centered provider of end-of-life care focused on supporting patients and their loved ones with comfort and compassion. The role includes following individualized plans of care, assisting with personal care, supporting families, providing attentive service, and maintaining accurate documentation.

Requirements

High school diploma or GED.
Current, unencumbered LNA license in the state where the employee works.
At least one year of experience as an LNA in an acute care or long-term care setting.
Current driver’s license and vehicle insurance.
Access to dependable transportation.

Responsibilities

Follow the patient’s plan of care to support comfort and well-being.
Respond promptly and compassionately to requests from patients and their families or caregivers.
Assist patients with bathing, oral hygiene, changing bedding and clothing, and shampooing.
Provide exceptional customer service to promote patient satisfaction.
Share knowledge and guide and support families caring for terminally ill loved ones.
Maintain accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
