About The Position

Brookhaven Hospice is seeking a full-time Hospice Aide (LNA) to provide hands-on care that supports comfort and dignity for patients at the end of life. Working closely with the nursing team, the LNA helps meet patients’ physical, emotional, and personal needs with respect, empathy, and professionalism. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., serving Bedford, Hooksett, Concord, and Manchester.

Requirements

• High school diploma or GED.
• Current, unencumbered LNA license in the state where the employee will work.
• At least one year of experience as an LNA in an acute care or long-term care setting.
• Current driver’s license and vehicle insurance.
• Access to dependable transportation.

Responsibilities

• Follow the plan of care to support patients’ comfort and well-being, and respond promptly to requests from patients and their families or caregivers.
• Assist with personal care, including bathing, oral hygiene, changing bedding and clothing, and shampooing.
• Provide exceptional customer service and work to ensure patient satisfaction.
• Share knowledge and expertise with families, guiding and supporting them in caring for terminally ill loved ones.
• Maintain accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
