About The Position

Brookhaven Hospice is seeking a full-time Hospice Aide (LNA) to provide hands-on care to hospice patients, helping bring comfort and dignity to patients at the end of life. The LNA works closely with the nursing team to address patients’ physical, emotional, and personal needs with respect, empathy, and professionalism. The position covers Concord, Hooksett, and Manchester, with a schedule of Monday through Friday, 8:00 a.m. to 4:30 p.m.

The role includes following individualized plans of care, assisting with personal care, supporting families, providing attentive service, and maintaining accurate documentation.

Requirements

• High school diploma or GED.
• Current, unencumbered LNA license in the state where the employee will work.
• At least one year of LNA experience in an acute care or long-term care setting.
• Current driver’s license and vehicle insurance.
• Access to dependable transportation.

Responsibilities

• Follow each patient’s plan of care to support comfort and well-being.
• Respond promptly and compassionately to requests from patients and their families or caregivers.
• Assist with personal care, including bathing, oral hygiene, changing bedding and clothing, and shampooing.
• Provide exceptional customer service and work to ensure patient satisfaction.
• Share knowledge and expertise with families and guide and support them in caring for terminally ill loved ones.
• Maintain accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
