Requirements

  • Associate Degree in Nursing (ADN)
  • Current RN licensed in Colorado
  • BLS certification/training and certification
  • Knowledge of scope and role the RN, LPN, MA
  • Intermediate knowledge and understanding of information technology associated with Electronic Health Records.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) is preferred.
  • Bilingual preferred.
  • 1 year of clinical experience is preferred

Responsibilities

  • Manage and lead daily patient flow
  • Triage patient and evaluate immediate treatment
  • Monitors team task boxes, follows-up and delegates to MA’s & C.N.A’s when appropriate
  • Answer patient questions regarding medication and conditions
  • Delegate to MA when interpretation is not anticipated
  • Primary contact for Pod Support in relation to Portal Direct Messages
  • Mentor clinical MA & nursing staff. Monitors and evaluates skills, provide real time feedback, training and support
  • Provide day-to-day direct supervision/coaching of Nurses and or Medical Assistant Team
  • Reports ongoing noncompliance of policy & procedures, initiatives and workflows to Nurse Coordinator.
  • Monitors and assists with the implementation of nursing policies and workflows.
  • Supervises medication management and immunization administration
  • Assists student preceptors, providing guidance, training and education for both preceptor and student when needed.
  • Assists the Regional Director of Nursing with the completion of onboarding training plans and staff performance evaluations.
  • Responsible for insuring Rapid Response paper work is completed and sent to Nurse Coordinator and Compliance Nurse.
  • Creates daily, weekly, monthly duties lists and assigns/delegates duties to team members
  • Responsible for corrective action for MA/nursing staff, in collaboration with the Nurse Coordinator and Clinic Manager, as needed.
  • Other duties as assigned
  • Assist nursing/MA staff with patient care as needed
  • Perform basic health-related tasks, such as documenting patient information, administering medications, drawing blood, taking vitals, starting an IV and responding to emergencies
  • Operate standard medical equipment
  • Process and analyze lab results
  • Communicate and assist medical providers, nurses, medical assistants, and other staff.
  • Provide direct patient care through independent nurse visits, and the use of nursing protocols
  • Scheduled Nurse Visits for education: Medication self-administration, glucometer use, self-use BP use (Home BP monitoring)
  • Educate patients on nutrition, exercise, and lifestyle recommendations based on medical history and current condition.
  • Schedule and complete patient co-visits per policy
