About The Position

Provide in-person psychotherapy and behavioral health services to older adults and medically complex adults in Skilled Nursing Facilities and Assisted Living communities throughout the Lowville area. The role offers the opportunity to build meaningful, ongoing relationships with residents while working independently with support from an established multidisciplinary team. Administrative and operational support is provided, allowing providers to focus on clinical care. Full-time and part-time schedules are available, with flexible scheduling.

Requirements

Hold an active and unrestricted New York license as a Clinical Psychologist, LCSW, LMHC, or LMFT.
Have a master's or doctoral degree in an applicable discipline.
Provide compassionate, patient-centered care.
Have strong communication and documentation skills.
Be comfortable providing services in nursing home and long-term care settings.

Nice To Haves

Experience working with geriatric populations.
Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or Assisted Living settings.
Interest in interdisciplinary and integrated healthcare.

Responsibilities

Provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities.
Conduct diagnostic assessments and develop treatment plans.
Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
Maintain timely and accurate clinical documentation.
Collaborate with interdisciplinary care teams to improve patient outcomes.
Build therapeutic relationships with residents and their families.
