About The Position

Provide in-person psychotherapy and behavioral health services to older adults and medically complex adults in Skilled Nursing Facilities (SNFs) and Assisted Living communities throughout the Cooperstown area. The role offers full-time or part-time work with flexible scheduling, clinical autonomy, established referral relationships, and administrative and operational support. Providers work with a multidisciplinary team and build ongoing therapeutic relationships with residents and their families.

The position is open to fully licensed Clinical Psychologists, Licensed Clinical Social Workers (LCSWs), Licensed Mental Health Counselors (LMHCs), and Licensed Marriage & Family Therapists (LMFTs). Only providers with an active, unrestricted New York license are eligible; limited permits, associate licenses, LMSWs, MHC-LPs, provisionally licensed therapists, and supervised or pre-licensure applicants are not eligible.

Requirements

• Active and unrestricted New York license as a Clinical Psychologist, LCSW, LMHC, or LMFT.
• Master's degree or doctoral degree in an applicable discipline.
• Ability to provide compassionate, patient-centered care.
• Strong communication and documentation skills.
• Comfort providing services in nursing home and long-term care settings.
• Only fully licensed providers are eligible; limited permits, associate licenses, LMSWs, MHC-LPs, provisionally licensed therapists, and supervised or pre-licensure applicants are not eligible.

Nice To Haves

• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.

Responsibilities

• Provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities.
• Conduct diagnostic assessments and develop treatment plans.
• Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams to improve patient outcomes.
• Build therapeutic relationships with residents and their families.
