About The Position

Provide in-person psychotherapy and behavioral health services to residents of Skilled Nursing Facilities and Assisted Living communities in the Rye, New York area. The role serves older adults and medically complex adults, with opportunities to develop ongoing therapeutic relationships while working independently and collaborating with an established multidisciplinary team. Administrative and operational support is provided so clinicians can focus on patient care.

The position is available full-time or part-time, with flexible scheduling. Only fully licensed providers are considered; limited permits, associate licenses, LMSW, MHC-LP, provisionally licensed therapists, and supervised or pre-licensure applicants are not eligible.

Requirements

• Hold an active and unrestricted New York license as a Clinical Psychologist, LCSW, LMHC, or LMFT.
• Have a master's or doctoral degree in an applicable discipline.
• Provide compassionate, patient-centered care.
• Have strong communication and documentation skills.
• Be comfortable providing services in nursing home and long-term care settings.
• Be fully licensed; limited permits, associate licenses, LMSW, MHC-LP, provisionally licensed therapists, and supervised or pre-licensure status are not eligible.

Nice To Haves

• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.

Responsibilities

• Provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities.
• Conduct diagnostic assessments and develop treatment plans.
• Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams to improve patient outcomes.
• Build therapeutic relationships with residents and their families.
