Requirements
- Licensed Social Worker (LMSW or LCSW) in Washington, D.C.
- Minimum of 1 year of clinical or medical social work experience preferred
- Strong communication, empathy, and problem-solving skills
- Knowledge of community resources and care coordination
- Ability to work independently and manage a flexible schedule
Nice To Haves
- Home health or healthcare experience is a plus
Responsibilities
- Assess patients’ social, emotional, and environmental needs
- Develop individualized care plans and resource strategies
- Provide counseling and emotional support to patients and families
- Assist with discharge planning and community resource coordination
- Connect patients to housing, financial, and support services
- Collaborate with nurses, therapists, and physicians
- Complete accurate and timely EMR documentation
