Daymark Health is a value-based oncology company offering personalized cancer care through care navigation, symptom-focused support, behavioral health care, social resources, evidence-based interventions, and a hybrid in-person and virtual care model. As the first person in this role, you will establish the foundation of Daymark’s care support program and make it easy for others to follow. A key focus is providing cognitive behavioral therapy (CBT) to patients diagnosed with cancer. You will become an expert in the Daymark care model and available community resources, conduct initial needs assessments, and create care plans that address gaps such as palliative care, advance care, site-of-service shifts, and end-of-life planning. During your first month, you will welcome members and provide regular individual sessions, screen for unmet clinical, medication, behavioral, safety, and psychosocial needs, and use and improve assessment tools and templates. You will connect members with community services and public benefits, contribute to maintaining the community resource database and care protocols, and document patient and caregiver interactions in the EHR. During your first three months, you will build trusting relationships with patients and caregivers, monitor side effects and changes in patient function or behavior, and support providers with medication management strategies. You will consult with licensed clinical team members, triage complex or medical issues as needed, and participate in weekly interdisciplinary patient rounds. This is a full-time, remote role, Monday through Friday, 8:30 AM to 5 PM PST.
