This multi-specialty psychology group offers general therapeutic and diagnostic services including treatment for mood and behavior disorders, relationship concerns, memory and cognitive concerns, academic problems, as well as personal growth and development. The role involves providing case management, care coordination, and planning for outpatient care settings. The social worker will assist patients, family members, and caregivers in receiving the most appropriate options and services to meet their complex healthcare needs, including acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. This position requires coordinating care with multiple providers across all levels and sites of care, addressing psychosocial as well as medical needs of patients and their families/caregivers through participation in multidisciplinary patient care management practice, and evaluating care and outcomes to ensure the most appropriate and effective care and services.
