About The Position
Provides case management and social work services by assessing individuals, developing care plans, coordinating resources and referrals, monitoring rehabilitation progress, advocating for clients, and providing crisis intervention.
Requirements
- Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs.
- Valid driver's license.
Responsibilities
- Assessing individuals, developing individual care plans and determining necessary resources.
- Advocating for and assisting individuals and families in accessing and obtaining services.
- Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals.
- Monitoring individual rehabilitation goals and progress.
- Providing crisis intervention services.
